3/4/2023 EnchantaVaughn Approved Behavior: Client exhibited good social behaviors, interacting well with others and demonstrating that she has a creative imagination, able to learn while sheâs multi-tasking in the home. While entering into the home setting, client was observed by QMHP cursing at her younger sibling and not completing chores within the home setting. Behavior: Client appears to be more aware of herself and what she has to do in order to obtain her objectives daily. During the second portion of the session, client was observed by QMHP throwing items at her siblings as she was getting upset with them while in the session. Intervention: QMHP will collaborate with parent/close family member regarding behaviors and performance. QMHP intervened in an effort to encourage client to make the right decision. QMHP encouraged client to make good choices and to utilize self-calming techniques (i.e. counting and deep breathing) and processed with her that this behavior is acceptable and discussed actions and consequences. QMHP will teach client necessary coping skills to decrease impulses. QMHP will brainstorm strategies with client and encourage self-monitoring. QMHP will provide mediation and relaxation techniques, as needed. Intervention: QMHP praised and recognized client for her hard work, efforts, participation, and working towards improvement by changing daily habits and implementing a positive and consistent routine. QMHP supported client with meeting her objectives and coping with stressors without needing additional redirection or prompts. QMHP provided client with positive reinforcement to encourage client to follow through with her good decision-making skills. QMHP encouraged client to practice relaxation and coping techniques in social/academic environment at home and in the community. Response: Client wants to make her family and parents proud by doing a good job every day. Client retained a lot of the information she was taught which motivates her to pay more attention and take her time. Client is observed problem solving on her own and handling her emotions appropriately. Client avoids meltdowns and lashing out when upset or frustrated. Client responds well to feedback. Client responds well to the positive reinforcement provided by QMHP and is seen increasing self-esteem and confidence. Response: Client have not implemented safe boundaries in social environment and is affected by her own failure to recognize personal dangers or control violent behaviors. Client need to work on all objectives listed on her ISP, and follow the interventions provided by mental health counselor and the rules set up by parent in the home. Client said she was going to practice coping strategies and relaxation techniques in all aspects of her daily living routine in hopes that she will be a better person, daughter, client, and friend.
